If you do not have a way to print them, we encourage you to arrive 15 to 20 minutes before your appointment to ensure you have the proper time to fill out forms in our office.

Same Day Emergencies
If you have a dental emergency, please call our office. While Akhrass Dental Implant & Cosmetic Center does not accept Walk-Ins, if you will please call first, we strive to treat as many same day emergencies as we can.
Our patient’s care is our top priority. Therefore, we have partnered with Care Credit and Proceed Finance (third party financing companies) to help our patients obtain the dental treatment and the smile that they deserve.
